Has anyone done this? I imagine it would be necessary to include a pier-type extension like the one Orion make - raises it by 8-9 inches to let the CWs clear the tripod legs. But mounting an EQ6 on a pier extension and still be able to do azimuth adjustments for polar alignment is what has me a bit stuck. Any thoughts - preferably with engineering drawings :D?

Sounds like an interesting idea...
I use the SW 200mm pier extension on the top of my fixed pier. Uses a 12.5mm central bolt.
The top plate of the extension holds the original stop pin for the Az adjustment, so any ALT-AZ adjustments are carried out as per normal as they would with the original SW tripod.

I modified surveyor-type tripod to accept LXD75, by adding adapter.. .. EQ6 is a bit bigger, that's all the difference there is.
(the setup on the image was used for imaging total Sun eclipse in Cairns)
My pier adapter (http://www.iceinspace.com.au/forum/showthread.php?t=84067&highlight=pier) may give you the idea for your project
